Forms Needed For Grades K-8

All new students and those entering the 4^th  grade in September, are required to have both the Archdiocese Immunization and Health Inventory on file before classes start in September.  Those students without these completed forms on file will not be permitted to enter class in September.

Authorization to Administer Prescription and Over the Counter Medication (if needed)

The AUTHORIZATION TO ADMINISTER PRESCRIPTION AND OVER THE COUNTER MEDICATION form must be completed each new school year by a parent/guardian AND a physician if you would like your child to receive any medications at school (even Tylenol or Ibuprofen).  Medication will not be administered during school hours unless this form is completed and signed by a physician. Please review the back of this form for information and procedures regarding the administration of medication at school.

Students are not permitted to carry any medication (this includes inhalers, throat lozenges, etc.) at school.
