FREE FLUMIST VACCINE CLINIC

Who can participate?:   Saint Elizabeth School Students/Alumni aged 18 years and younger

What do I need to do to sign-up ?:

·      Review the attached Fact Sheet with important information you must know before signing the consent form.

·      Fill out, sign, and return the attached consent form by Wednesday January 4, 2011.

·      Send an email to bchilds@stelizabethschoolmd.org to reserve a flumist vaccine in the clinic for your child/children.
