Dear Parents and Guardians,                                                                                                                                                                          

Although the peak in H1N1 has passed, the virus is still in our community and there remains some chance of a 3rd wave this spring.  Montgomery County Department of Health and Human Services still has a supply of the mist vaccine available.  They have made it available to our Saint Elizabeth Student community. We will be holding a free nasal mist H1N1 vaccine clinic on Monday, March 29^th.

If you would like your child to receive a dose of the nasal mist, please print out, complete, and sign the attached consent form.                               

Please return the completed form to the Health Room no later than Wednesday March 17.
